Instructions for using printable registration form:

1. Print and complete form. (Download the PDF)
2. Individuals and Organizers, make sure you cut off the “receipt” section of the form. Organizers must complete the contact areas on the receipt and give to registrants.
3. Scan or send picture of registration form to register@apspuhuru.org. Make sure image can be clearly read.
4. Mail registration form with payment (check, money order, credit card) information to:

  1245 18th Avenue South
  St. Petersburg, FL 33705.
  Make check payable to “Uhuru”

You will receive a confirmation from the registrar once form and payments are received. If doing installment payments print form each time to submit payment.
